Rilonacept is an engineered dimeric fusion protein
consisting of the ligand-binding domains of IL-1 receptor (IL-1R) and IL-1
receptor accessory protein (IL-1RAcP) linked to human IgG1.Rilonacept blocks IL-1 beta signaling by
acting as a decoy receptor that binds IL-1 beta.
Rilonacept, approved by the FDA under Orphan Drug Status in February 2008, is the first approved biologic in the new class of therapeutics known as 'Cytokine Traps' . These 'traps' are engineered to exhibit much higher binding affinities to their targets than traditional decoy receptor constructs which possess only monomeric binding sites (e.g. anakinra).
Treatment of
cryoprin-associated periodic fever syndromes (CAPS), including Muckle-Wells
syndrome (MWS) and familial cold autoinflammatory syndrome (FCAS) in children
12 years and older are approved FDA indications.
